Healthcare information is kept about all patients of Oxleas, including health problems and care and treatment received. These records are stored for our assessment requirements.

The General Data Protection Regulation says you have the right to see the information we hold about you. This is known as ‘right of subject access’. Therefore, you are allowed to see any records we hold about you.

If you would like to request to see your healthcare records, please download and complete the request form below. More information about your rights and how we look after your information can be found in our Privacy Statement which is available to download in our Privacy policy section.

If you want to request a copy of your medical records or require other information please email the Information Governance Team on oxl-tr.igteam@nhs.net. To make a freedom of information request please email oxl-tr.FOI@nhs.net.  Due to the current public health situation regarding COVID-19, phone calls or letters will be monitored on a reduced basis but will be responded to as soon as possible.
