Supporting you to live well with COPD, heart failure and diabetes

Chronic obstructive pulmonary disease (COPD), heart failure and diabetes are long term conditions. This means that people with these conditions are likely to have them for a long time, for most, this means the rest of their life.

We've developed this section of our website to support you to live well with your long term condition, as well as to support your family and friends to better understand and help you manage your condition. You'll find information related to diet and exercise, medication, support services, self management and much more. Information is separated into conditions:

If you are registered with a Greenwich GP then you can call our Central Access Team on 020 8320 3550 (Monday to Friday from 8.30am to 5pm) to book routine appointments.

If you currently use our Greenwich COPD, Heart Failure or Diabetes services and are feeling unwell or you need to speak to the service urgently, please call:                                                               

  • COPD team on 020 8320 3340 from 9am to 5pm, Monday to Saturday
  • Heart Failure service on 020 3260 5237 from 9am to 5pm, Monday to Friday
  • Specialist Diabetes Team on 020 3260 5104 from 9am to 5pm, Monday to Friday.  

Outside of these hours, call GRABADOC on 020 8319 3030 or if your condition is serious or life threatening, call 999. 


If you are registered with a Bexley GP and use Oxleas diabetes, heart failure or COPD services, please call 020 8320 3345 for COPD and heart failure enquiries or 01322 357 902 for diabetes enquiries.
