Joint Emergency Team Greenwich

The Joint Emergency team assesses people, aged 18 years and over, in their own home to:

  • Reduce unnecessary hospital admissions
  • Crisis intervention
  • Plan and arrange rehabilitation and ensure it is undertaken in the appropriate setting ie in their own home or in an intermediate care facility, with the patient’s best interests at heart. 


The Joint Emergency team accepts referrals from all primary, community, acute and social services. This service is available to Greenwich residents or those registered with a Greenwich GP. Referrals are made through the Joint Emergency Team.

Greenwich Adult Community Health Services’ Joint Emergency Team is based at the Woolwich Centre.

Oxleas Patient Experience survey

The Oxleas Patient Experience survey gives you the opportunity to feedback on the care and treatment you have received. We also encourage your friends, family and carers to also provide feedback. Hearing your views gives us a better understanding of the needs of our patients, families and carers which helps us to make sure that we are providing a good standard of care. 

The short survey only takes a few minutes to complete and you can provide a written comment explaining what you think is working well and what is not. The Patient Experience team will collect the results of the surveys and these are shared with the relevant departments and management teams. This helps us to shape our services to continually improve the quality of the care and treatment we provide.

Sabrina and her team have provided a fantastic service and support for my 82yr old mother in law. She was discharged from hospital 24hrs after a hip replacement with no thought given to how she would manage at home.

Sabrina visited promptly, provided equipment the same day, gave kindness, guidance and practical advice on everything she needed to know, and gave my mother in law the confidence to progress through the first stages at home. We are so grateful for her care and support. Without this service, we would not have been able to manage . A very big thank you.
