Lynda cares for her 89 year old mother Joan who has a number of long term health conditions.
“Before Marite contacted me I had to make 10-15 calls just to coordinate my mum’s care, at a time when she was having frequent admissions to hospital. Now, thanks to Oxleas, I just have to make one.”
Marite, the care navigator from Greenwich Coordinated Care, now provides a single point of contact for Lynda and organises the care that Joan needs.
“This could include contacting a GP, social worker, district nurse or specialist nurse. She also sends me useful information, for example how to get a ramp so that I can get my mother’s wheelchair into my car.”
Weekly calls from Marite also provide peace of mind and allow Lynda to concentrate on caring for her mother.
“People don’t realise the pressure on a carer and just having someone to talk to is so important.”
