Children's Community Nursing Service Bexley

The Children's Community Nursing Team provides a range of services to support children and young people who require skilled nursing support in their own home, school, clinic or a community setting. They are a team of specialist children's nurses and healthcare assistants who provide a range of services for children with complex, chronic or acute healthcare needs.

What we offer

High quality nursing service to enable early, safe planned discharge from hospital and to prevent unnecessary admissions.

To act as a link between primary and secondary care and support and encourage the development of partnership working.

The care will be delivered by fully trained and competent professionals working in partnership with the children and families in the location most suitable.

To support children and young people with acute, chronic and complex health care needs to live as full a life as possible. Providing a flexible, comprehensive service that meets the physical, emotional, spiritual and social needs of the children and young people, their families and carers.

To help the children achieve their full potential by working in partnership with them, their family, education, children’s services and other voluntary organisations.

Who do we see?

The Children’s Community Nursing Service assess and manage the needs of children aged 0-19 years with:

  • acute and short-term conditions
  • long-term conditions
  • disabilities and complex conditions, including those requiring continuing care and neonates
  • life-limiting and life-threatening illness, including those requiring palliative and end-of-life care.

The child should be registered with a Bexley GP.

What do we do?

The Children's Community Nursing Team provide specialist nursing care, support and co-ordination of care to children and young people with a range of complex health care needs and disabilities. These include those with life limiting/life threatening conditions, chronic illnesses, palliative care needs, end of life care needs and short term acute care needs.

The Children's Community Nursing Team work in partnership with acute and community paediatricians, therapists, GP’s, Health Visitors, School Nurses, Schools, Children’s Services and voluntary organisations to help the children and young people achieve their full potential and live as full a life as possible. This care is delivered in the community location that is the most appropriate for that child or young person e.g home, school respite centre.

The Children's Community Nursing Team team also provides nursing support into the Special Schools Service Hours - Monday to Friday 9.00 am – 5.00pm.

The Children’s Continuing Care team provide individualised packages of care to children and young people with long term, complex health needs, many of whom are technology dependant. This care is provided around the clock and all receiving the service must meet the criteria for children’s continuing care.

What happens next/on your first appointment?

Following referral, contact is made within 24 hours, an appointment booked for a convenient time, and a holistic assessment undertaken by a member of the nursing team. A care plan is agreed and the nurse will ask other relevant professionals/ therapists to contribute to the plan.

How to find us:

The hospital is beside the A20 Sidcup Bypass, and you should come off at Frognal Corner onto the A222. Queen Mary’s Hospital has 350 pay and display public parking spaces with bays for people with disabilities outside each main building.  There is a pay and display car park next to Acorns.
