Stroke Rehabilitation Service Bexley

The Stroke Rehabilitation Service aims to ensure the safe and timely discharge of patients from the stroke units at Princess Royal University Hospital in Farnborough and Queen Elizabeth Hospital in Woolwich. The service is designed to provide a seamless transfer from hospital to home and enable stroke survivors to receive stroke specialist rehabilitation in their own homes.

The service is also available, via GP referral, to Bexley patients who are about to be discharged from the stroke unit at Darent Valley Hospital.

The team will contact you within 24 hours of being discharged from hospital to arrange an assessment of your rehabilitation needs. The team will then develop a treatment programme with you. The duration and intensity of this will be based on your personal needs and will last up to 13 weeks. If you require further rehabilitation after this time, it will be provided by the Community Rehabilitation team.

The team is made up of occupational therapists, physiotherapists, speech and language therapists and rehabilitation assistants. Dietetics, psychology and nursing support is also available through the team if required.
