Community Assessment and Rehabilitation Greenwich

The Community Assessment and Rehabilitation (CAR) teams work in partnership with Greenwich Council to provide community-based assessment, rehabilitation and prevention services for people over 18 years of age.

Our team includes:

  • physiotherapists
  • occupational therapists
  • community nurses
  • therapy assistant practitioners

Services provided include:

  • Individual and group exercise programmes
  • Balance and exercise classes
  • Onward referral to Strong and Steady for ongoing support for balance
  • Mobility practice, activity and task practice
  • Assessment of movement and provision of walking aids
  • Assessment of daily living activities
  • Home hazard assessments
  • Provision of equipment to assist independence following an assessment of the home environment
  • Provision of advice and assistance to carers.

Home visits are available for those who are unable to attend clinics or who require a home environment assessment.

There are clinics at:

  • Greenwich Square Health Centre
  • Eltham Community Hospital
  • Gallions Reach Health Centre

Referrals can be taken from any health and social care professionals.
