Make a referral

You can refer to speech and language therapy, occupational therapy and physiotherapy using this form. You only need to complete the sections relevant to the service you are referring to.

Early Years/pre-school referrals

  • The Early Years Service sees children from birth to the end of nursery
  • Children must be registered with a Greenwich GP
  • Anyone can refer to the service, including parents/carers
  • Please complete all sections of the referral form, in as much detail as possible. This helps us triage the referral accurately. Incomplete referral forms will be returned.

School Years referrals

  • The School Years Service sees young people from reception age, up to 19 years old (25 with an education, health and care plan (EHCP)
  • Anyone can refer, however we recommend the referral is discussed with your school's special educational needs coordinator (Senco) first
  • Some of our services are based at the school that a child attends. Please ensure this information is included on the referral form
