London region lead learns about Oxleas neighbourhood care

NHS England London Region lead visit Oxleas NHS Greenwich Frailty Team

Members of the Greenwich Frailty Team enjoyed sharing the positive outcomes of their Value Improvement Project with Dame Caroline Clarke, Regional Director for NHS England London, when she visited them at Memorial Hospital last week.

Dame Caroline met many members of the team and heard how their proactive and connected care with partners has reduced the need for hospital treatment for their patients and costs have been reduced by reducing unnecessary medication. She was impressed by how the team works closely with partners in the borough through sharing care planning and also links patients into community groups to promote their health and wellbeing.

Oxleas CEO Ify Okocha was delighted to welcome Dame Caroline to Oxleas and to share the good practice that the teams demonstrate. He said:

At Oxleas, we're committed to working in partnership to benefit local communities. Keeping people well for as long as possible and treating them as close to their homes as possible is vital and we are working to develop the infrastructure to support this further.

The London lead also heard about the work of the Bexley Integrated Complex Case Team through a presentation from team manager Suki Basi. Again, Dame Caroline was impressed with the proactive work with care homes in the borough which helps to keep people well for longer and reduce unnecessary trips to hospital.

Lisa Cooper, Acting Joint Service Director, Adult Community Physical Health Services, shared information on how the team works as part of BexleyCare. She said:

It was wonderful to welcome Dame Caroline to Oxleas adult physical health services. The visit was a great opportunity to showcase the excellent work of our teams and how integrated working is delivering better outcomes for patients while supporting a more joined‑up and effective system.

Filming and photography requests

We receive documentary, location and broadcast filming requests on a regular basis – we consider all proposals on an individual basis and will try to accommodate proposals where possible, if appropriate.

No photography or filming is permitted without prior arrangement and agreement. Our patients always come first and we have a duty to protect their safety and confidentiality. Anyone found filming or taking photographs on the trust premises without the permission of the trust’s communications team will be asked to leave by the security department and may face prosecution. 

Unless agreed otherwise, film crews and photographers must be accompanied by a member of the Communications Team at all times.
