As a provider of NHS community physical and mental health services, we fully support the shift from hospital to community and believe developing our partnership working in neighbourhoods further will benefit local communities. We will now focus on how we can put these plans into action.

Neighbourhood health centres will house services under one roof, open at evenings and weekends. The Plan sets out how the NHS will be transformed to train thousands more family doctors, transform hospital outpatient appointments, and provide personalised care plans for complex needs.
The ambition is to have millions of patients treated and cared for closer to their home by new teams of health professionals.
Neighbourhood health centres will provide easier, more convenient access to teams of nurses, doctors, social care workers, pharmacists, health visitors, palliative care staff, and paramedics. Community health workers and volunteers will also play a key role in these teams.
