Using digital technology to improve care

Patients are using digital technologies at home more often, working with health and social care, to live more independently and improve quality of life.

Research indicates that keeping people out of hospital and helping them live independently for longer to recover in their own homes significantly improves mental and physical health. Patients prefer this approach and it helps with their recovery to be treated in the comfort of their own familiar surroundings.

Bringing together a range of support in a person’s own home prevents unnecessary hospital admission, or helps people return home sooner - whatever home means to them.

In partnership with Oxleas NHS, the Royal Borough of Greenwich and South East London ICS, technology for use at home can provide early intervention, support self-care, help with everyday living activities and ensure people feel safe and supported at home.

The technology will be available to a wide range of people of all ages including those being discharged from hospital, those who are frail and at risk of falls, and people with dementia.

Staff are trained to use and demonstrate new technologies, which have been co-designed by staff and local residents.
