The District Nursing Service provides planned and unplanned care for adults aged 18 and over who cannot leave their home. Care is offered 24 hours a day, 7 days a week.

We aim to help you live as independently as possible and work together with you, your family, carers and other health and social care professionals. We always treat you as an individual and create a care plan that meets your needs.
To access the service, patients must be registered with a GP in the borough of Greenwich.
You can be referred to the service by:
Self-referrals are accepted from patients already known to the District Nursing Service and also accepted from patients who have been under the care of the District Nursing Service for the same nursing need within the previous 12 months.
Please note that self-referrals will not be accepted regarding medication and you will need to discuss this with your GP. To self-refer, please call 020 8320 3550.
