The Source

The Source is a community clinic based in the Horn Park neighbourhood.
It's staffed by an advanced clinical practitioner nurse who delivers various clinical activities including support with long term conditions, like reviewing blood pressure and blood glucose checks, wound care, medication advice and minor illness and injuries. We also provide services for children.

Service hours:
Temporary closure and relocation of The Source.The Source will close from Thursday 30 April 2026 to allow refurbishment work to take place.From Tuesday 5 May 2026, services will continue from a mobile clinic unit in the Horn Park Community Centre car park, located just across the road from the current site at 65 Sibthorpe Road, Horn Park, Lee, SE12 9DN.Advanced Nurse Practitioner walk‑in clinics will still be available. Specialist sexual health services will not run during this time. If you need these services, you will be given advice and details of other services you can use.
Our team
We are a service staffed by:
Tuesday and Thursday mornings: An advanced clinical practitioner nurse, specialising in long term conditions
Tuesday and Thursday afternoons: Live Well coaches from Charlton Athletic Community Trust.
Our aims
We aim to provide access and signposting to community healthcare services closer to home for residents of the Horn Park Estate and surrounding area.
How to access this service
To access this service you must:
- Live in the Horn Park neighbourhood, the Greenwich borough or registered with a Greenwich GP
- Be aged 18 or above (except our sexual health services)
The service is for people who have long term conditions, we hold morning clinics on Tuesday and Thursday with an advanced nurse practitioner.
Referrals
No referral is needed, this is a drop-in service and patients are assessed in order of arrival.
How to contact this service
The Source is open Tuesdays and Thursdays.
From 9am to 1pm residents can see an Advanced Nurse Practitioner specialising in long term conditions, with the last walk in at 12.30pm.
