Adult Autism Service – Autism Partnership Programme (APP)

  • The Autism Partnership Programme (APP), previously known as Transforming Care Programme, are a specialist team of multi-disciplinary professionals that work with autistic clients, community mental health teams and acute mental health services.

Our team

Our team consists of:

  • Clinical and Counselling Psychologists
  • Occupational Therapists
  • Speech and Language Therapists
  • Assistant Psychologist
  • Specialist Support Worker
  • Administration Officers

Our aims

The Autism Partnership Programme (APP) aims to improve health and care services for autistic people so that more autistic people can live in the community with the right support close to home.

Our aim is to reduce the risk of admission to hospital and expedite discharge for people who have already been admitted.

How to access this service

We work with:

  • Individuals who are over 18 and/or those supporting them with a confirmed diagnosis of Autism Spectrum Disorder (evidenced by a NICE approved report).
  • Do not have a learning disability or forensic background (or do not meet eligibility criteria for these services).
  • Are at risk of or currently admitted to a mental health ward/unit.
  • Currently open to a mental health team in Oxleas.
  • Registered to a GP in the London Boroughs of either Bexley, Bromley or Greenwich.

Referrals

Referrals can be made by staff from our inpatient or mental health teams.

We're open Monday to Friday, 9am until 5pm. Please note we only offer appointments during those hours.
