Chronic Obstructive Pulmonary Disease (COPD) Service - Bexley

  • The Chronic Obstructive Pulmonary Disease (COPD) Service provides specialist care to patients with COPD. The team helps patients manage their condition and stay as healthy as possible. To do this we use a combination of clinic appointments and home visits.

    The COPD specialist nurses can provide information and advice to:

    • Make sure you understand your diagnosis
    • Explain your condition and symptoms to your family
    • Reassure you about changing symptoms which can affect your lifestyle and make you feel afraid
    • Help you to manage your condition and be independent
    • Discuss treatment and care options as your condition changes.

How to access this service

To access this service, you must be:

  • Aged 18 or above
  • Registered with a Bexley GP
  • Have a COPD diagnosis
  • Require oxygen therapy
  • Referred for spirometry

We do not accept referrals for asthma.

Referrals

You can be referred to this service by:

  • Your GP
  • Other healthcare professionals

Referrals are made via the central access team on: oxl-tr.centralaccessteam@nhs.net. We first assess all patients by telephone. We then arrange a face-to-face appointment in your own home or at a clinic.

Patients are then admitted to our caseload and discharged on completion of treatment. Patients can be referred back to the service at any time by a healthcare professional.

All patients are given our contact number and can call anytime. The service is available from 9 am to 5 pm, Monday to Friday, excluding bank holidays. You can leave a message outside these hours and we will call back within two working days.
