Community Mental Health Rehabilitation and Enablement Service (CMHRES) - Bexley

  • The Bexley Community Mental Health Rehabilitation and Enablement Service (CMHRES) is a community-based mental health team that provides intensive support to people with severe and enduring mental health difficulties.

    All of the patients we work with are in a rehabilitation process and would otherwise be in hospital.

Our aims

We aim to:

  • Support service users to recover, rehabilitate, and remain in independent accommodation within the local community
  • Help service users lead active and enjoyable lives, free from isolation, poverty, abuse and physical ill health
  • Prevent relapse and admission to hospital through frequent contact, a consistent approach to medication management, involvement in care planning, and psychosocial support

Our team

We are a multidisciplinary team. This means we are a group of professionals who specialise in different areas of medicine but work together to make decisions about how to care for service users.

Our team is made up of:

  • Administrative staff
  • Medics
  • Nurses
  • Occupational Therapist
  • Psychologist
  • Student nurses
  • Support workers

Our service is based on the Intensive Case Management and Recovery models and CQC (Care Quality Commission) standards. We abide by Oxleas NHS Foundation Trust policies and the Care Programme Approach (CPA).

How to access this service

To access this service, you must be:

  • Under the care of an Oxleas team and have a Care Coordinator who can provide full and up-to-date RiO data. This includes a Core Assessment, Risk Assessment and Care Plan.
  • Able and willing to maintain contact with medical and mental health services and accept Bexley CMHRES support to work towards rehabilitation.
  • Able to establish two rehabilitation goals at the point of assessment.

Referrals

We do not accept referrals for people with:

  • A primary diagnosis of autism or learning disability
  • A primary diagnosis of substance use
  • A primary diagnosis of personality disorder
  • Symptoms which occur as a result of neurological disorder. For instance, an organic condition, a head injury or a neurological impairment resulting from chronic alcohol or substance misuse

Mental health problems often coexist with other difficulties, such as substance abuse. This will not be used as a reason for exclusion from the service.

However, if there are significant co-existing difficulties, the team will offer expertise and advice and work closely with the lead care provider.

Likewise, severe mental illness and personality problems can coexist and present in very similar ways. The presence of personality-based features is not a reason for exclusion from the CMHRES team. In these cases, the team takes its time to assess whether the patient will benefit from this service. During this time, we work with the referring team to provide assistance.

Referrals need to contain the following information:

  • RiO number
  • Reason for referral
  • Rehabilitation needs
  • Anticipated outcomes
  • Strengths and goals of the service user
