Community Rehabilitation and Short-Term Assessment Team - Greenwich

  • The Community Rehabilitation and Short-Term Assessment Team (CRSTAT) works together with Greenwich Council. They provide community-based assessment, rehabilitation and prevention services for people over 18.

    Our services include:

    • Individual and group exercise programmes
    • Balance and exercise classes
    • Onward referral to Strong and Steady for ongoing support for balance
    • Mobility practice, activity and task practice
    • Assessment of movement and provision of walking aids
    • Assessment of daily living activities
    • Home hazard assessments
    • Provision of equipment to assist independence following an assessment of the home environment
    • Provision of advice and assistance to carers

    We can visit your home if you are unable to come to one of our clinics.

How to access this service

To access this service, you must:

  • Be aged 18 or above
  • Live in Greenwich or are registered with a Greenwich GP
  • Require support to live independently
  • Have a goal you want to achieve and are willing to work towards it

  • A health and social care professional
