Complex Case Team Service - Bexley

The Complex Care Service works to identify, discuss and coordinate care for Bexley residents with high-risk and complex needs. We aim to ensure patients can safely remain in their own homes or care homes.
To do so, we work closely with primary care, physical and mental health, social care, the voluntary sector and other agencies.
The service focuses on identifying complex health and social care needs as early as possible. This allows for better management of those needs.
Our aims
We aim to improve health and well-being outcomes and prevent emergency admission to hospitals in the future.
How to access this service
To access this service you must be:
- Aged 65 years or over
- Registered with a Bexley GP. This includes patients living in nursing homes and residential care homes
Referrals
You can be referred to this service by:
- Your care home
- Your GP
- Your GP’s LAS
- Other healthcare professionals.
We do not accept referrals for:
- Individuals under the age of 65
- Individuals in an acute crisis.
All referrals will be discussed within an MDT to ensure we provide the appropriate care and support.
