Referrals
Patients registered with a Riverview PCN GP surgery can be referred by any health or social care professional for an assessment at home. This includes the following surgeries:
- Conway Surgery
- Ferryview Surgery
- Gallions Reach Health Centre
- Royal Arsenal Medical Centre
- St Marks Medical Centre
- Thamesmead Health Centre
