Heart Failure Services - Bexley and Greenwich

  • The Bexley and Greenwich Heart Failure service helps patients actively manage heart conditions and provides education and support. This includes drug titration to patients with a diagnosis of heart failure that is confirmed by an echocardiogram.

    The service is mostly delivered in community clinics. Home visits are available for patients who are housebound.

Our team

We are a multidisciplinary team. This means we are a group of professionals who specialise in different areas of medicine but work together to make decisions about how to care for our patients.

How to access this service

To access this service, you must be:

  • Registered with a GP in Bexley or Greenwich
  • Aged 18 or above
  • Diagnosed with heart failure with reduced ejection fraction
  • Assessed by your GP or acute sector as requiring the service

We do not accept referrals for:

  • Heart failure patients with preserved ejection fraction and no diastolic or valvular disease
  • Patients with renal dialysis heart failure
  • Patients under the care of Her Majesty’s Prisons

Referrals

Referrals must include:

  • Relevant medical history
  • Current medication
  • Most recent blood test results
  • Echocardiogram
  • Discharge summary if the patient was recently admitted to hospital

If a referral is assessed as inappropriate, it will be discharged. We will inform the referrer of our decision.
