Joint Emergency Team (JET) - Greenwich

  • The Joint Emergency Team (JET) assesses people who are aged 18 and over in their own homes.

    We do so to:

    • Reduce unnecessary hospital admissions
    • Help in moments of crisis
    • Plan and arrange rehabilitation
    • Make sure rehabilitation takes place in the right setting and with the patient’s best interests at heart

    JET Navigator Nurse

    • The Navigator Nurse helps identify patients who do not need admission to hospital or can be discharged earlier from hospital as their needs can be met in the community.
    • The Navigator Nurse is part of a multidisciplinary team. This is a group of professionals who specialise in different areas of medicine but work together to make decisions about how to care for patients.
    • The Navigator Nurse takes person-centred approach. They identify appropriate care pathways that mean treatment, support and care is provided in the patient's own home or an intermediate care facility.
