Our aims
We aim to help you be more independent in your daily life. We can work with you, your family and your carers and we come to you to do our work. That means we can visit you at home, in your day centre, at work, or as an outpatient or inpatient at Queen Mary’s Hospital.
There are specialists on our team who can help with specific issues, like returning to work.
Our team
Our team includes:
- Physiotherapists
- Occupational therapists
- Speech and language therapists
- Nurses
- Therapy assistant practitioners
- Specialist multiple sclerosis and Parkinson’s Disease nurses
We work closely with other local services in Bexley and neighbouring areas. These include GPs, acute hospitals, leisure services, social services, the Stroke Association and other patient organisations.
How to access this service
To access this service, you must:
- Be aged 16 or above
- Live in Bexley
- Be registered with a GP in Bexley
- Have a neurological condition
- Can participate in rehabilitation. This means you know what you want to work on and are willing to do exercises that will help you reach those goals.
Referrals
You can be referred to our service by:
- Your hospital
- Your GP
- Another agency you work with
We are not an emergency service. If you need help urgently, please get in touch with your GP. We aim to contact you within seven days of your referral.
Our service is open seven days a week. For telephone queries, you can contact us Monday to Friday, from 8.30 am to 4.30 pm.
