We aim to help you be as independent as possible in your daily life. To do this, we work with you wherever is easiest. This can be in your home, in your day centre or at work. We can also work with you, your family, friends or carers.
We are a multidisciplinary team. This means we are a group of professionals who specialise in different areas of medicine but work together to make decisions about how to care for our patients.
