When referred to this service, the referral team will contact the ward manager. They will provide the service user’s full clinical background.
The Oaktree team then assesses the patient’s case and decides whether Oaktree Lodge can provide suitable care. We will always inform the referral team of our decision.
The Oaktree team requests confirmation of funding from local commissioners. Once we receive confirmation, we begin planning for admission with a CPA handover. The service user and Care Coordinator will be invited to visit Oaktree Lodge to meet staff and become familiar with the ward.
