Our aims

We aim to prevent unnecessary visits to Accident and Emergency and admissions to hospitals for individuals living in Bexley.

 

Our team

We are a multidisciplinary team. This means we are a group of professionals who specialise in different areas of medicine but work together to make decisions about how to care for our patients.

The team is part of a planned and integrated whole-system approach to care. This means we work with a range of other primary and secondary services to deliver support and care.

How to access this service

To access this service, you must:

  • Live in the borough of Bexley or be registered with a Bexley GP
  • Be aged 18 or above
  • Be medically stable
  • Require a rapid health and or social care assessment to prevent A&E attendance or hospital admission
  • Require an intensive home care assessment and support to prevent emergency admission to hospital or residential settings
  • Require bed-based or community-based rehabilitation

We do not accept referrals for people who:

  • Are medically unstable
  • Have acute or active substance misuse issues
  • Have an acute mental health condition

Referrals

The Rapid Response Team aims to maximise accessibility by offering an open referral policy. We receive referrals from:

  • All services within primary and secondary care
  • Local authorities
  • Members of the public

Referrals are received via the main base at Queen Elizabeth Hospital.

The team will respond to urgent referrals within two hours. We follow up with patients in the community within five days to prevent the condition worsening and readmission to the hospital.
