The Bexley Rapid Response Service is a specialist service for individuals suffering sudden deterioration in their health and social care needs.
We aim to prevent unnecessary visits to Accident and Emergency and admissions to hospitals for individuals living in Bexley.

The team is part of a planned and integrated whole-system approach to care. This means we work with a range of other primary and secondary services to deliver support and care.
