Single Point of Contact Service - Bexley

Our Single Point of Contact (SPC) Service provides one point of contact for all new referrals from GPs in the borough of Bexley. It covers referrals for the following services:
- District Nursing
- Phlebotomy
- Adult Safeguarding
- Adult Social Care
- Heart Pathway
- Community Assessment and Reablement Pathway
- Neuro
- Continence Pathway
- Diabetes Pathway
- Podiatry Pathway
- Respiratory Pathway
- Complex Wound Care
- Lymphoedema Pathway
- Integrated Case Management (ICM)
- Adult Mental Health
- Early Intervention Psychosis
- Older People’s Mental Health and Memory Pathway
The service manages both health and social care referrals. It works alongside other service teams to make sure every referral reaches the right service.

The SPC service operates at:
Bexley Civic Offices
Monday to Friday (8 am - 6 pm)

Saturday to Sunday and Bank Holidays (8am - 5pm)
At weekends, the team operates a reduced service and will process referrals for District Nurses. Electronic referrals for other services will be picked up when the service re-opens.
Out-of-hours service information will be available after 6 pm.
