Have your say on community-based care plans

The NHS in South East London wants to hear your views on further development of community-based services as alternatives to having to be admitted to hospital.

People often find it hard to know where to go to access care, and part of this approach is to develop a central place through which people are referred to services – a single point of access or care co-ordination hub.

Services being further developed as part of this work include virtual wards and urgent community response teams.

If you have experience of community-based care through either of these, then please take the time to share your views via a short survey, which closes on 10 November.

Your feedback and recommendations will be used to look at what works well and what can be improved on.
