The Bromley Community Learning Disability Team (CLDT) provides care and support to adults with learning disabilities whose needs cannot be met by universal services. We also support those people who care for adults using the service.
The team achieves this by:
- Assessing the needs of adults with learning disabilities.
- Clarifying eligibility for services.
- Providing advice and education about patient health and healthy lifestyles.
- Providing therapy or treatment for mental health problems, challenging behaviour, emotional problems, mobility and epilepsy.
- Advising on independence, self-help skills, communication skills, continence and medication.
- Advising and supporting patients with long-term health conditions, including dysphagia and diabetes.
- Providing excellent community care to avoid hospital admissions.
- Supporting discharge planning from hospitals to prevent readmissions.
- Advising on short-term and long-term life planning.
We also work alongside an Intensive Community Support Team (ICST). The ICST provides intensive support for people with learning disabilities when there is a risk of hospital admission or placement breakdown. The ICST also offers support to carers, families and support staff.
The ICST is a small psychology team that works with people who are open to the Community Learning Disability Teams in Bexley, Bromley, and Greenwich.

We are a multidisciplinary service. This means we are a group of professionals who specialise in different areas of medicine but work together to make decisions about how to care for our patients. The team includes:
- Psychologists
- Psychiatrists
- Community Nurses
- Occupational Therapists
- Physiotherapists
- Speech and Language Therapists
How to access this service
To access this service, you must:
- Be registered with a Bromley GP
- Be over the age of 18
- Have a Moderate or Severe learning disability or a Mild learning disability with additional complexities
- Have complex health needs
