Older Adults, Community Mental Health Service - Bexley

  • The Older Adults Community Mental Health Service provides mental health services for older adults (aged 65 and over) in the borough of Bexley.

We offer support and help older adults to achieve their personal goals.

We review care plans regularly so that people can live as independently as possible.

Following assessment, the team will discuss what action to take. This may involve a referral to psychology, an outpatient review or a follow-up with a nurse or occupational therapist.

All referrals not accepted by the team, are either transferred back to GP or signposted to other services.
